The U.S. Antarctic Program (USAP) Physical Qualification (PQ) processes
The USAP PQ process is broken down into the full process or a short-duration process, which is determined by the University of Texas Medical Branch (UTMB) or NSF.

1. Packet receipt: UTMB receives packet request. PQ packet (forms and instructions) are emailed to recipient.
2. Packet completion: Participants complete required documentation, testing and examinations as identified in the original email.
3. Packet submission: PQ packet is submitted via fax or mail as outlined in the initial PQ email.

4. Initial review: Upon receipt, medical and dental records are reviewed by UTMB administration. Missing items are identified and requested.
5. Follow up: Additional documentation and/or missing items are requested by administration.
6. Final Submission: Documentation is faxed or mailed. Once determined packet is complete, it is forwarded for medical review.

7. Physician and dental reviews: Packet reviewed by physician (medical and labs) and dentist (dental and x-rays.) Medical/dental concerns are identified that may affect qualification based on the criteria in the PQ Guidelines. Additional information/documentation may also be requested in this step.
8. Initial determination: Participant receives either a Physical Qualification (PQ) or Not Physically Qualified (NPQ) notification in writing.
9. Waiver process: If NPQed, a waiver can be requested (directions are provided in the determination letter.) Once required information is received it will be forwarded to NSF for final review and determination.

1. Packet receipt: UTMB receives packet request and hiring or supervisory authority confirms/requests short duration eligibility (see SOH Policy.) PQ packet (forms and instructions) are emailed to recipient.
2. Packet completion: Participants completes medical history and infectious disease requirements.
3. Packet submission: PQ packet submitted via fax or mail as outlined in the initial PQ email.
